The FOP needs to update your information. Please take the time and fill out the form and send the form to the FOP office. The FOP needs to have your current information.
Click below and use one of the forms. The online form, allows you to fill out the blocks and print the form. The printable form, allows you to print the form and write in the blanks. Use either form and send the form to the FOP office. When using the online form, if you do not see square information blocks, you cannot use the form on the computer that you are using.
